Design, layout, and content mean everything to making Google Adsense pay off. I was using the Leaderboard (728 x 90) and the Wide Skyscraper (160 x 600), and was getting low CTRs.

I then got rid of the Leaderboard, kept my Wide Skyscraper, and added the Large Rectangle (336 x 280). I placed the Large Rectangle underneath the article title, and above the article content. I also removed the borders, and used a link color that's similar to my site's navigational link color. I immediately saw my CTR skyrocket. Below are examples that you can look at.

Also, I believe keeping a clean site, and making your site load quickly (well written without a lot of images) will help your website go a long way towards getting found on search engines, and having people visit your site and click on ads.

Do you think that you are losing first-time visitors who are turned off by seeing ads before seeing content?

Personally, I would say no. First of all, I think people are use to header ads, and Google ads in particular. Secondly, being that google ads are usually quite relevant to site's content, I think that many people consider them valuable resouce links in their search for what ever they are looking for. As long as your using the competive ad filter and not loosing your visitor to a major rival, it only makes sense to put the ads where they are most effective if your intent is to profit from your visitors through your advertising.

Have you tried using the same size in a Left aligned table so your content is also there right at the beggining? I still have to try this out, but have seen a few sites use it.
