When I run my adWords ad throughout the world, I get a CTR of 4%, but when I run the same ad in the United States, I get a CTR of 0.5%. That is a big difference that I am not able to explain. What do you think may be happening?

I agree, its the type you visitors (interest group) that you have on your site. Try ad variations with different content 9 blocking some of the sites with less clicks, refresh the screen and look at the ads , you will find out the companies) and that way you might know which site will get what type of clicks.
