it's a good thing you stated this because this is the step that's usually done wrong, no offense meant for other SEO's here but from the planning process they go wrong, how come with the other processes. Especially if they didn't understand the need of their client

I totally agree to you that the only way yo stay visible in the world wide web is through SEO! to add up, there are two types of SEO.. first is On Page and the second one is OFF Page.. the steps you stated above are mix of what SEO is.. Here i will give you some guide on optimizing a site, On Page SEO Guide.. and the other one is about OFF Page(link building, etc...).. these are just basics.. but it will be a big help for sure..
