I'm using www.100webspace.com hosting. They offer free plans with php/mysql, provided that you have their banner displayed on your website; plan without banners costs $1.99 per month if I'm not mistaken; and there are a few paid plans with lots of extra features.

I've been with them for over a year now, and am quite happy with the service. Customer support is lightning fast, as for downtime they only had it a couple of times but servers were up in no time.
